HMDA requires that you collect an applicant’s demographic information (DI) at the time of application. This includes providing the disclaimer below prior to requesting them to provide their DI (race, sex and ethnicity) as well as the options (main categories and subcategories for each) for them to self-identify. If the application is taken over the phone; however, this information must be read to them. Appendix B to §1003 #1 states, …for applications taken by telephone, you must state the information in the collection form orally, except for that information which pertains uniquely to applications taken in writing, for example, the italicized language in the sample data collection form.
